ZGaetano e22cf625bf feat(panel): add disconnectFromServer(); toggle connection-form / connected-bar 
- On connect success: hide form, show compact connected-bar with hostname
- On disconnect: clear assets, reset buttons, restore form
- Wire disconnect-btn click to disconnectFromServer()
